You must ensure your portfolio is correctly composed using the content requirements and templates listed below for 3 yearly PDRP portfolios.

Competent portfolio requirements

1. Portfolio assessment tool (PAT)
2. Application letter
3. Copy of APC (print profile from NCNZ)
4. Role description
5. Full self and peer assessment (full performance review)
6. Evidence of practice hours
7. Professional development career plan
8. Professional development record
9. Curriculum Vitae (optional)

Proficient portfolio requirements

Capital & Coast completed portfolios for application to Accomplished level must be received by the Nurse Coordinator - Professional Development by the 1st of the month to be assessed by the panel that month.
